Comments:
A renewal inspection was conducted today, September 22, 2023. The licensing inspector was onsite from approximately 10:00 am until 12:00 pm. The assistant director was on site and participated with the inspection. Later the director arrived. There were 16 children present, ranging in ages from seven months to five years with four staff supervising. Children were observed building with magnetic blocks and legos and working puzzles. Infants were playing with toys on the floor. Later children listened to a story and prepared for lunch. Children provide their own lunch which were labeled with their name and date. Diapering and handwashing was observed.
The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, and medication. A total of seven children's records and five staff members? records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and a violation was documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-160-A
Description: Based on review of five staff records, the center did not obtain documentation of a tuberculosis screening for one staff record that was submitted at the time of of employment and shall have been completed within the last 30 calendar days prior to employment.

Evidence:

The record of staff #1, employed 09/11/2023, contained documentation of a tuberculosis screening dated 04/10/2023 exceeding 30 calendar days prior to employment.

Plan of Correction: Per the director: We will ensure Tuberculosis screenings are collected at the time of employment and are no older than 30 days.

Standard #: 8VAC20-780-60-A-8
Description: Based on review of seven children?s records, the center did not have a written allergy action care
plan for one child diagnosed with a food allergy, to include instructions from a physician regarding the steps to take in the event of an allergic reaction.

Evidence:

The record for Child #6 (enrolled 01/12/2022) indicated the child has a diagnosed food
allergy. A written food allergy action plan had been obtained however it was not signed by a physician.

Plan of Correction: Per the director: The parent submitted a note stating she did not have the food allergy any longer. I will have the parent provide a not from the doctor as the child's physical states she does have a food allergy.

Standard #: 8VAC20-780-270-A
Description: Based on observations and interview, the center did not maintain all areas and equipment in a clean, safe, and operable condition.

Evidence:

1. The wooden gate separating the two playgrounds had a protruding nail and split board creating a jagged edge that could entangle or snag the skin of a child in care.
2. The director stated she was aware of the broken fence picket.
3. The metal cellar door on the playground has peeling paint and rust. The door is accessible to children in care.

Plan of Correction: Per the director: My husband has been working on the fence. He has completed half of it and will continue the repairs as soon as the weather clears. We will repaint the cellar door.
