Comments:
A monitoring inspection was conducted on August 18, 2023. There were 169 school age children present in the center, with 22 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of ten child records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the plan of correction and the date to be corrected for each violation listed on the violation notice and return it to the department within 5 business days from receipt. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s); 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures.

Standard #: 8VAC20-780-130-A
Description: Based on a review of children's records, the center did not ensure that each child's record contained documentation of immunizations required by the State Board of Health before the child can attend the center.

Evidence:
The record for child #10 had documentation of an immunization record without a name or date of birth.

Description: Based on a review of children's records, the center did not ensure that each record contains all required information.

Evidence:
1. The records for child #4, child #9, and child #10 did not contain the address for one of the two designated people to call in an emergency.,
2. The record for child #7 did not have two designated people to call in an emergency.

Description: Based on observation, the center did not ensure areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.

Evidence:
1. In cabin 10, 11, 12, 13, 14, and 15 the electrical panels are without covers.
2. In the pool bathrooms one hole behind the second toilet.
3. In the basement of the lunch area one floor tile is missing in the hallway.

Description: Based on observation and staff interview, the provider did not ensure prescription medication was be labeled with the child?s name, the name of the medication, the dosage amount, and the time or times to be given.

One medication, an Epi-Pen was observed without a prescription label with the child?s name, the dosage amount and the time or times to be given

Standard #: 8VAC20-780-510-P
Description: Based on a review of records, the provider failed to notify a parent after a medication needed to be picked up within 14 days, and then failed to dispose of the medication that was not picked up.

Evidence.
A non-prescription allergy medication for child #6 expired April 2023. The allergy medication was still at the center. A member of management acknowledged that a parent was not notified to pick up expired medication and the center did not dispose of the expired medication.
