Comments:
A monitoring inspection was conducted on 09/25/2023. There were a total of 39 children with 3 staff members present. Children were observed playing games and watching a movie. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 3 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Violations:
Standard #: 8VAC20-780-245-J-1
Description: Based on a review of staff records, the supervisor did not ensure medication administration practices were current.
Observation:
Staff #1 and Staff #2 had Medication administration training that expired on 6/22/2020.

Plan of Correction: MAT certifications had expired, however staff were enrolled in the next MAT class which was 9/26/2023 (next day)

Standard #: 8VAC20-780-510-F
Description: Based on a review of medication authorization, the supervisor did not ensure required documents were available for emergency medication.
Evidence:
Emergency medication was on site for child #6 and child #7 without proper authorization.

Plan of Correction: Multiple email, text and in person reminders were sent to the parent requesting the authorization forms. Was told there was a back up at the doctors office. Finally received forms at 2:15pm on 9/25/2023 via email.
