Comments:

A monitoring inspection was initiated on 9/22/23 and concluded on 9/22/23. There were 20 children present with 9 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed. The inspection started at 9:30am and concluded at 11:35am. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #:	8VAC20-780-160-A
Description:	Based on review of staff files the center failed to ensure that tuberculosis tests or screenings were completed within the last 30 calendar days of the date of employment. Evidence: Staff #1 had a TB document dated 04/2023, the listed hire date for this staff was 07/2023. The TB document was completed outside of the required 30 days of hire.
Plan of Correction:	Staff will obtain a new TB test or screening for compliance. We will ensure the TB document is not over the allowed 30 days prior to hire requirement for future hires.
Standard #:	8VAC20-780-160-C
Description:	Based on review of staff files the center failed to ensure that tuberculosis testing or screenings are done every two years from the date of the initial testing or screening. Evidence: 1. Staff file #2 had a TB document dated 7/23/21. 2. Staff file #3 and #4 had a TB document dated 9/21/21. 3. Staff file #5 had a TB document dated 7/8/21. TB testing or screenings are to be re-done every 2 years.
Plan of Correction:	Staff will obtain a new TB test or screening. A spreadsheet will be made with expiration/renewal dates of items to ensure future compliance.
