An unannounced monitoring inspection was conducted on September 26, 2023 between the hours of 10:15am - 12:15pm. There were a total of 55 children with eleven staff members present. Children were observed during snack time, centers, handwashing, book reading, and engaged in group activitie. A selection of staff and children records, medications and other documents were reviewed. The physical space, emergency supplies, evacuation drills, attendance records, and the Emergency Preparedness Plan were reviewed. Areas of non-compliance are identified in the violation notice. Standard #: 22.1-289.035-A
Description: Based on record review, the center did not update criminal history record reports every five years.
1. Staff #3's record (Date of hire-08/28/2017) did not contain documentation of a repeat criminal history check. Documentation of the previous criminal history check results was dated 06/01/2018.

Plan of Correction: The staff member will make an appointment to complete fingerprints.

Standard #: 22.1-289.035-B-2
Description: Based on record review, a criminal history check was not completed prior to the first date of employment.
1. Staff #1's record (Date of hire-08/21/2023) contained documentation of a completed criminal history check after the first date of employment. The criminal history results were dated 09/11/2023.

Plan of Correction: Fingerprints for staff members will be completed prior to the first day of employment.

Standard #: 8VAC20-780-70
Description: Based on record review, required records were not kept for each staff person.
1. Staff #1's record (Date of hire-08/21/2023) contained documentation of two or more reference checks completed after the first date of employment. The reference checks were dated 08/27/2023 and 09/01/2023.

Plan of Correction: Moving forward, administration will ensure that reference checks are completed prior to staff's first date of employment.
