Technical Assistance:
Consultation provided on documenting the time the monthly escape drills are being conducted, making sure you are providing orientation training on all components, please remind all staff that diapering areas should remain free of objects due to cross contamination and consultation provide on furniture location and being able to easily access a child in a crib.

Comments:
An unannounced monitoring inspection was conducted today with the director from 2 pm to 3:55 pm. Sixty nine children were present with 8 staff providing supervision. The ages of the children ranged from 4 months to 5 years of age.

Most of the children were napping during the walkthrough, however some of the babies were awake and being diapered or on the floor and being read to by staff. The children in the transitional JK program were busy with a variety of activities.

Six staff and five children's files were reviewed. A sample size of medications for 6 children were reviewed as well as 3 allergy care plans. The fire inspection was dated 1/18/23 and the health inspection was dated 1/17/23.

The areas of non compliance are outlined on the violation notice.

Violations:
Standard #: 8VAC20-780-330-B
Description: Based on observation, it was determined that the playground did not maintain an adequate resilient surfacing around playground equipment and did not maintain adequate fall zones.
Evidence:
1. On the older playground the Licensing Inspector observed divots in the poured-in-place resilient surfacing. One divot was located 2 feet in front of the end of the single tan slide with the other divot located 4 feet in front of the end of the double tan slides.
2. On the younger playground the Licensing Inspector observed a portable blue and green slide within 4 feet of the Awning pole on 3 sides and within 5 feet of the chain link fence on the 4th side. The standard requires this slide to have a 6 foot clearance from all obstacles.

Plan of Correction: We will remove the portable slide on the younger playground and place the tan slides on the older playground off limits until we can repair the poured in place.
