Comments:
An unannounced monitoring inspection was conducted on September 27, 2023 from approximately 10:35 am to 11:35 am. Two staff were present with two preschool age children. Children were observed playing freely and doing table activities. Three staff and two child records were reviewed. Violations were found and are noted on the violation notice.

Violations:
Standard #: 8VAC20-780-150-B
Description: Based on review, facility did not ensure 1 of 2 child records has physical examination and immunization signed by a physician, designee or an official of a local health department.

Evidence: The record for child #2 (start date 12/6/2022) has a physical examination and immunization record that was not signed by the physician, designee or health department official.

Plan of Correction: Director will have parent get a signed copy of the physical and immunization record.

Standard #: 8VAC20-780-60-A
Description: Based on review, facility did not ensure 2 of 2 child records have documentation of required information.

Evidence: The following was found during record review:
1. The record for child #1 (start date 9/18/2023) did not have parent's employment phone number and a signed parent agreement.
2. The record for child #2 (start date 12/6/2022) did not have allergy information.

Plan of Correction: Paperwork will be sent home with child today to have parent complete.

Standard #: 8VAC20-780-330-B
Description: Based on observation, facility did not ensure there is at least 6' of fall zone with adequate resilient surfacing around climbing equipment or equipment with moving parts on the outside playground.

Evidence: There was approximately 3.5' of fall zone between the two plastic play sets with slides and approximately 4' of fall zone between the see saw and fence.

Plan of Correction: The equipment will be moved to allow at least 6' of fall zone around each piece of play equipment that children climb on or have moving parts.

Standard #: 8VAC20-780-540-E
Description: Based on observation, facility did not ensure the required nonmedical emergency supplies are available.

Evidence: There were not sufficient batteries available to operate the flashlight and radio.

Plan of Correction: Batteries will be purchased and kept available for the radio and flashlight.
