Technical Assistance:
Discussed proof of identity documentation. Discussed raking the mulch after it rains or when the mulch is pushed away. Discussed the safety belts on the infant table.

Comments:
An unannounced monitoring inspection was conducted on 9/28/2023 from 10:00am to 12:08pm. At the time of entrance, 30 children were in care with 8 staff members present. Children were observed playing with toys, having a group discussion about shapes and preparing to make a color form art project, playing with legos, coloring, and playing with playdough. Infants were observed sleeping in cribs, sitting in a bouncy seat, being held and drinking a bottle and one was playing on the floor. A selection of (3) staff and (3) children records, medications, the physical space, evacuation drills, allergy action plans and attendance records were reviewed. The site was clean and organized and contained an abundant supply of developmentally appropriate materials. Bulletin boards and several other areas of the programming space was decorated with artwork and projects completed by the children. Interactions between the children and staff were positive. Areas of non-compliance are identified in the violation notice.

Please specify how the deficient practice will be or has been corrected. Please do not use staff names; list staff by positions only.

Description: Based on observation, the violation notice and inspection summary from the last inspection was not posted in a conspicuous place on the premises.
Evidence:
1. The inspection violation notice and summary from 8/08/2023 was not posted.

Plan of Correction: I will post the inspection today.

Standard #: 8VAC20-780-130-E
Description: Based on review, the center did not obtain documentation of additional immunizations once every six months for children under the age of two years.
Evidence:
1. Child #2 (start date 4/01/2022) was under the age of 2 years old and had the last updated immunization record on 8/02/2022 at their yearly appointment. An update was needed approximately February 2023.

Plan of Correction: I will ask them for the updated immunization record.

Based on record review, three children's records did not have complete information.
Evidence:
1. Child #1 (Start date 11/01/2021) was missing two parent's work phone numbers, one emergency contact address, previous child cares attended and the annual update.
2. Child #2 (Start date 4/11/2022) was missing two work addresses, previous child cares attended and annual update.
3. Child #3 (Start date 3/20/2023) was missing one work address.

Plan of Correction: I will ask the parents for the information today and updates.

Based on record review, staff records did not include all required information.
Evidence:
1. Staff #2 (Date of hire 4/18/2023) had references completed on 7/01/2023 and 7/05/2023, but it was not completed prior to hire.

Description: Repeat Violation
Based on observation, Based on observation, equipment of the center outside were not
maintained in a safe and operable condition.
Evidence:
1. Three push cars were missing parts or broken. One was missing the steering wheel. The other one was missing the steering wheel and ladder part on the right side and the last one had a broken piece to the right of the steering wheel and was missing the door.

Plan of Correction: Replace the toys on the playground.

Standard #: 8VAC20-780-280-B
Description: Based on observation, hazardous substances were not kept in a locked place using a safe locking method.
Evidence:
1. In the Lion's classroom, a spray bottle of bleach water was under the sink. The lock was hanging down on the cabinet handle and not engaged or locked.

Plan of Correction: Make sure to tell the teachers that cleaning supplies need to be locked.
