An unannounced monitoring inspection was conducted on 9/27/2023 from 4:00pm-5:30pm. At the time of the inspection there were 3 children in care and 2 staff members present. The record sample size consisted of 5 children?s records and 3 staff records. The inspector toured the facility and observed classroom activities. Children were observed engaged various activities and snack. Emergency supplies, evacuation drills and required postings were reviewed during the inspection. The information gathered during the inspection determined non-compliance with applicable standards or law and were documented on the violation notice and discussed during the exit interview.

Standard #: 8VAC20-780-130-A Description: Based on record review and interviews, it was determined the center did not ensure that documentation was obtained showing that each child in care had received immunizations as required by the State Board of Health before the child attends the center.

Standard #: 8VAC20-780-160-A Description: Based on a review of 3 staff records, it was determined that the facility did not ensure that all staff provide documentation of a negative tuberculosis (TB) screening at the time of employment and prior to coming into contact with children.

1. The record for staff #1 (start date 9/25/2023) did not contain documentation of a negative TB screening.

Standard #: 8VAC20-780-280-B Description: Based on observation and interviews it was determined the center did not ensure that hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children.
Evidence:
1. In the classroom there was a bottle of cleaning solution in an unlocked cabinet, accessible to children.,
2. Staff #1 confirmed the clean solution was in an unlocked cabinet.Plan of Correction: Staff #1 immediately removed the bottle and placed it in a locked cabinet.
