Technical Assistance:
Today we discussed the procedures to confirm absence of a child when the child is scheduled to arrive from another program. These procedures, though included in the initial staff orientation training, may need to be posted at sites to ensure staff consistency and follow through. This process should occur in a timely manner, immediately following attendance tracking when children arrive after public school is dismissed.
All staff should understand this policy and readily demonstrate their knowledge of it during an inspection.


In order to accommodate the 4-year-old children in your ratios, please ensure that there is a clear separation of these children in a group, with their own assigned staff, at a 1:10 ratio of staff to children.
The school-aged group may then operate with another distinct group and their own assigned staff with a higher staff / child ratio.
All staff should understand this arrangement and readily demonstrate it during an inspection.

Comments:
A monitoring inspection was initiated on and concluded on 9/21/2023. There were 47children, ages 4 to 10 years old, present with 3 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 3 (all) staff records were reviewed. The inspection started at 3:30 pm and concluded at 5:30 pm.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program

Violations:
Standard #: 8VAC20-780-130-A
Description: Based on a review of records and interview, the licensee did not document having seen that each enrolled child has received the immunizations required by the State Board of Health before the child can attend the center.
Evidence:
Five children's records were reviewed.
The record for child #1, whose enrollment date was 9/11/2023, did not contain documentation that child #1 has received the immunizations required by the State Board of Health.
The site director confirmed that she did not have documentation of child #1's immunization record.

Plan of Correction: The program director will speak with the parent of child #1 today and request that the child's immunization record be submitted as soon as possible, but not later than 2 weeks from today (10/5/2023).

Standard #: 8VAC20-780-60-A-8
Description: Based on record review and interview, the licensee did not ensure that a written care plan is maintained for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.

Evidence:
Five children's records were reviewed.
The record for child #1 documented that this child has a dairy allergy.
The record for child #2 documented that this child has an egg allergy.
The record for child #3 documented that this child has a dairy allergy.
The records for child #1, #2, and #3 do not contain a written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.

The program director confirmed that this documentation was not available for child #1,child #2 or Child #3 during the inspection.

Plan of Correction: The program director has requested this documentation from the parents and will follow up on the request today as parents arrive to pick up their children.
The parents of child #1, #2 and #3 will be asked to submit this documentation no later than 10 days from now (10/5/2023).
