8VAC20-780-80 A written list of attendance should be with each group of children.

8VAC20-780-500B3 Children under the age of 3 should have diaper changes on a nonabsorbent diapering surface located on either a changing table or designated counter for changing.

8VAC20-780-560G All food from home, including water bottles if filled at home, will need to be labeled with name and date.

Comments:
An unannounced renewal inspection was conducted on 09/25/2023 from 9:30am to 12:25pm. There were 47 children ages 2 years old to age 5 years old supervised by 6 staff. The physical plant, outdoor playground area, programming, attendance, menus, 5 staff records, 5 children?s records, emergency drills, emergency supplies, and policies were reviewed. There were no medications on site. Children were observed participating in math activities, literacy activities, art, fine motor activities and outside play. Handwashing procedures were also observed. There was an adequate number of staff present with current certification in Medication Administration Training (MAT), CPR and First Aid, and Daily Health Observation (DHO) training. Areas of non-compliance are identified in the Violation Notice.

Standard #: 22.1-289.035-B-2
Description: (Repeat Violation) Based on review of 5 staff records, the provider did not obtain documentation of the results of a national fingerprint background check prior to date of hire.

Evidence: The record for Staff #2 (Date of Hire 6/1/22), did not contain documentation of a fingerprint background check completed prior to their date of hire. The fingerprint background check on file was dated 6/21/22.

Plan of Correction: Revise communication to potential employees about due dates for required documents.

Standard #: 8VAC20-770-60-B
Description: Based on review of 5 staff records, the center did not obtain documentation of a completed sworn disclosure statement for each staff prior to date of hire.

Evidence: The record for Staff #2 (date of hire 6/1/22) did not contain documentation of a completed sworn disclosure statement prior to the date of hire. The sworn disclosure statement on file was dated 6/4/22.

Description: Based on review of 5 staff records, the center did not obtain documentation of a negative tuberculosis (TB) test or screening for staff at the time of employment and prior to contact with children.

Evidence: The file for Staff #2 (date of hire 6/1/23) did not contain documentation of a negative tuberculosis (TB) test or screening at the time of employment and prior to contact with children. The negative tuberculosis (TB) test or screening contained in the file is dated 9/11/22.

Standard #: 8VAC20-780-240-A
Description: Based on review of 5 staff records, the center did not ensure that the Virginia Department of Education-sponsored orientation course was completed within 90 calendar days of employment.

Evidence: The record for Staff #2 (date of hire 6/1/2022) did not contain documentation that the Virginia Department of Education-sponsored orientation course was completed within 90 calendar days of employment. The documentation of the completed training was dated 9/25/23.

Plan of Correction: Certificate supplied on 9/27/23.

Standard #: 8VAC20-780-245-A
Description: Based on review of 5 staff records and staff interview, the center did not ensure that staff complete annually a minimum of 16 hours of training appropriate to the age of the children in care.

Evidence: The record for Staff #1 contained documentation of only 9 hours of annual training and the record for Staff #2 contained documentation of only 8 hours of annual training.

Plan of Correction: Training requirement fulfilled.

Standard #: 8VAC20-780-260-B
Description: Based on documentation and staff interview, the center did not ensure that they receive annual approval from the health department or approvals of a plan of correction, for meeting requirements for water supply, sewage disposal system and food service if applicable.

Evidence: The center did not have an inspection completed from the health department within the last year. The last inspection documentation provided was dated 3/29/22.

Plan of Correction: The health inspection was conducted on 10/3/23.

Standard #: 8VAC20-780-350-C
Description: (Repeat Violation) Based on observation, the center did not maintain the required supervision ratio of staff to children whenever children are in care.

Evidence:
During the inspection, the center did not maintain the required supervision ratio of 1 staff for every 8 children in the yellow house classroom where the youngest child was age 2.

Plan of Correction: Will be corrected the day after the child turns 3.

Standard #: 8VAC20-780-550-D
Description: Based on review of documentation, the center did not ensure they implement a monthly practice evacuation drill.

Evidence: There was no documentation for an evacuation drills for May of 2023.

Plan of Correction: Drill sheets have been revised to include all types to ensure all drills are recorded accurately.

Standard #: 8VAC20-780-560-F
Description: Based on observation and staff interview, the center did not ensure that a menu listing foods to be served for meals and snacks during the current one-week period was given to parents and kept on file for one week at the center.

Evidence: In the Yellow house classroom Staff stated there was no menu given to parents for the week of 9/18/23 or the week of 9/25/23.

Plan of Correction: All classrooms will follow notification system on TC. Discussed with staff on 10/4/23.
