Comments:
An unannounced monitoring inspection was conducted on 9/26/23 from 8:30am to 12:20pm with one of the Directors. There were 144 children in care initially, ranging in age from three-months to five-years-old, supervised by 22 staff. More children and staff arrived throughout the inspection. The children were observed eating a morning snack of waffles or pancakes and milk, reading books and having free play, having diapers changed, playing with play-doh, playing ?hot potato?, participating in circle time, and dancing. Six child records and five staff records were reviewed. The Center has all staff current in CPR and First Aid, and two staff trained in Daily Health Observation Training. Thirteen medications and authorization forms were reviewed and the center has 10 staff current in Medication Administration Training (MAT). Required postings were observed. The attendance, menu, and emergency drill log were reviewed. The most recent Fire Inspection on file was dated 11/9/22 and the most recent Health Inspection on file was dated 9/1/23. Please complete the "Plan of Correction" and "Date to be Corrected" areas on the Violation Notice for each violation cited and return to me by close of business on 10/6/23. Plans of correction should include steps to correct the noncompliance with the standard, and measures to prevent the noncompliance from occurring again.

Violations:
Standard #: 8VAC20-780-160-A
Description: Based on review of five staff records, the program did not ensure that each staff member submitted documentation of a negative tuberculosis (TB) screening at the time of employment and prior to coming into contact with children, and that documentation was completed within the last 30 calendar days of the date of employment. Evidence: The TB screening on record for Staff #1, with an employment date of 1/24/22, was dated 2/5/22. The TB screening on record for Staff #2, with an employment date of 6/9/23, was dated 6/17/23. The TB screening on record for Staff #3, with an employment date of 6/12/23, was dated 10/29/21. The TB screening on record for Staff #4, with an employment date of 5/31/23, was dated 6/21/23.

Plan of Correction: Under the new regulation, we will ensure that each new staff will obtain their TB test/screening by the end of orientation before having contact with children.

Standard #: 8VAC20-780-240-A
Description: Based on review of five staff records and interview, the program did not ensure that the Virginia Department of Education-sponsored orientation course (Preservice) was completed within 90 calendar days of employment by each staff. Evidence: The record for Staff #5, with an employment date of 11/11/19, did not include documentation of the Preservice training, and the Director confirmed that Staff #5 has not yet completed the training.

Plan of Correction: Staff #5 finished her Preservice training.
