Comments:
An unannounced monitoring inspection was conducted on 9/27/23 from 11:30am to 1:20pm with the Director. There were 31 children in care, ranging in age from three-years to four-years-old, supervised by four staff. The children were observed participating in circle time, listening to a recording of the song/book ?A Wonderful World?, taking a class picture for ?red day?, and transitioning between structured centers. Four child records and four staff records were reviewed. The Center has six staff with current certification in CPR and First Aid, as well as six staff trained in Daily Health Observation. A sample of eight medications and authorization forms were reviewed and the center has two staff current in Medication Administration Training (MAT). Required postings were observed. The attendance and emergency drill log were reviewed. The most recent Fire Inspection on file was dated 5/19/22 and the most recent Health Inspection on file was dated 1/4/23. Please complete the "Plan of Correction" and "Date to be Corrected" areas on the Violation Notice for each violation cited and return to me by close of business on 10/6/23. Plans of correction should include steps to correct the noncompliance with the standard, and measures to prevent the noncompliance from occurring again.

Violations:
Standard #: 22.1-289.035-B-4
Description: REPEAT Based on review of four staff records, the program did not obtain the results of a search of a criminal history record check from each state that a staff member resided in within the previous five years, prior to employment. Evidence: The record for Staff #3, with an employment date of 9/8/23, contained documentation that the staff member had resided in New Hampshire within the previous five years, but did not contain a criminal history record check from New Hampshire prior to employment.

Plan of Correction: Will obtain out of state checks as soon as possible.

Standard #: 8VAC20-780-160-A
Description: Based on review of four staff records, the program did not ensure that each staff member submitted documentation of a negative tuberculosis (TB) screening at the time of employment. Evidence: There were no TB screenings on record for Staff #2, with an employment date of 8/15/23; Staff #3, with an employment date of 9/8/23; and Staff #4, with an employment date of 8/18/23.

Plan of Correction: Follow up with staff.

Standard #: 8VAC20-780-70
Description: Based on review of four staff records and interview, the program did not obtain all required documentation for staff records. Evidence: The record for Staff #2 contains documentation that the staff member is a Lead Teacher, but the record does not contain documentation to demonstrate that the staff possesses the education required by the job position.

Plan of Correction: Obtain documentation.
