Comments:
A monitoring inspection was initiated and concluded on 9/27/2023. There were 82 children present with 16 staff supervising .The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. Children were observed eating lunch, transitioning to rest time and sleeping. Infants were observed sitting in bouncy seats, being held and bottle fed, crawling and having tummy time on the rugs and playing with toys. A total of 9 child records and 8 staff records were reviewed. The inspection started at 11:40 am and concluded at 2:55 pm.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Repeat violations were found and are identified within this report

Violations:
Standard #: 8VAC20-780-60-A-8
Description: *** REPEAT VIOLATION***
Based on a review of records and interview, the facility did not have a written care plan for each child with a diagnosed food allergy to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.
Evidence: The record for child #9 whose record contained documentation of an allergy to peanuts, did not contain a written care plan with instructions from child #9's physician in case of a reaction. The center confirmed that the allergy was documented in the child's record, but that a care plan was not present at the center for child #9. The same documentation was missing from child #9's record when it was reviewed on 5/24/2023.

Plan of Correction: The program director will speak with child #9's parent today at pick up and repeat her request that an allergy plan be documented for child #9. The program director will require that the plan be submitted to the center within 14 days.

Standard #: 8VAC20-780-520-A
Description: Based on observation and interview, the licensee did not ensure that nonprescription skin products are not kept or used beyond the expiration date of the product.

Evidence:
One tube of nonprescription diaper ointment (labeled with child #10's name) in the infant classroom had a printed expiration date of 8/1/2023.
Infant staff confirmed that the diaper ointment had expired.

Plan of Correction: The ointment will not be used and will be returned to the parent at the end of the day when child #10 is picked up from care.
