Comments:

An unannounced renewal inspection was conducted on 09/27/2023 from 6:10am to 7:15am. There were 16 children ages 5 years old to age 10 years old supervised by 4 staff. The physical plant, programming, attendance, menus, 7 staff records, 4 children?s records, 4 medications with corresponding authorization records, emergency drills, injury reports, emergency supplies, and policies were reviewed. Children were observed participating in art and games. There was an adequate number of staff present with current certification in Medication Administration Training (MAT), CPR and First Aid, and Daily Health Observation (DHO) training. Areas of non-compliance are identified in the Violation Notice. Please complete the columns for "Plan of Correction" and "Date to be Corrected" for each violation cited on the Violation Notice, and then return a signed and dated copy to the licensing office by 5pm on 10/6. Description:	Based on review of 7 staff records, the Center did not obtain a criminal history background check every five years from the date of the last criminal history background check. Evidence: The records for Staff #5, Staff #6 and Staff #7 did not contain a criminal history background check every five years from the last criminal history background check. The previous criminal history check was dated 9/20/18.
Plan of Correction:	Regional manager is working on it.
Standard #:	8VAC20-780-80-A
Description:	Based on review of the written attendance record and interview with staff, the center did not ensure they maintained a written record of daily attendance for each group of children documenting the arrival and departure of each child in care as it occurs. Evidence: During inspection, there were 16 children present, but the written record reflected there were 5 children present.
Plan of Correction:	Remind staff to fil out rosters as students enter the program.
Standard #:	8VAC20-780-510-H
Description:	Based on review of 2 medications, the center did not ensure that nonprescription medication be in the original container with the direction label attached. Evidence: The medication for Child #5 was not in the original container with the direction label attached, instead it was stored in a plastic baggie.
Plan of Correction:	Contacted the child's parents and they have brought in original container.
