Comments:
A monitoring inspection was initiated and concluded on 9/27/2023. There were 25 children present with 3 staff supervising at the time of the inspector's arrival. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. Children were observed playing tabletop games, having an afternoon snack, and practicing a fire drill. A total of 5 child records and 3 staff records were reviewed. The inspection started at 3:40 pm and concluded at 5:20 pm.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Repeat violations were found and are identified within this report.

Violations:
Standard #: 8VAC20-780-130-A
Description: Based on a review of records and interview, the licensee di not obtain documentation that they have received the immunizations required by the State Board of Health before the child can attend the center.
Evidence:
Five children's records were reviewed.
The records for child #1 and child #2, who both enrolled 9/5/2023, did not contain documentation of immunizations.
Staff confirmed that this documentation had not been obtained for child #1 and child #2.

Plan of Correction: Staff will contact the parents today and require that documentation of immunizations for child #1 and child #2, be submitted by 10/11/2023.

Standard #: 8VAC20-780-60-A
Description: *** REPEAT VIOLATION***
Based on a review of records and interview, the licensee did not ensure that a record containing all of the required information is kept for each enrolled child.
Evidence:
Five children's records were reviewed.
The record for chilld #1 did not contain an address for either of the two required emergency contacts in case the child's parent cannot be reached in an emergency.
Staff verified that this information was not documented for child #1.

Plan of Correction: Staff will speak to the parent this week and request the addresses for the emergency contacts listed in child #1's record.

Standard #: 8VAC20-780-60-A-8
Description: ***REPEAT VIOLATION***
Based on a review of records and interview, the licensee did not obtain a written care plan for each enrolled child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.
Evidence:
Five children's records were reviewed.
The record for child #1contained documentation of a food allergy to butternut squash.
The record for child #5 contained documentation of a food allergy to mustard.
The records for child #1 and Child #5 did not contain a care plan to follow in the event of a suspected or confirmed allergic reaction.
Staff acknowledged that this documentation had nor been obtained for child #1 or child #5.

Plan of Correction: Staff will speak to the parents today and request that a written care plan, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction, be obtained and submitted for child #1 and child #5.
The care plan will be required by no later than10/11/2023.
