Technical Assistance:
Reviewed designated program space and provider verified the second floor is not being used as part of the designated space.
Discussed in detail the requirements for outdoor equipment related to resilient surfacing. The playground was not inspected due to inclement weather.

Comments:
An unannounced monitoring inspection was started in the center on September 27, 2023 from 10:55 am until 1:00 pm and concluded on September 27, 2023. At the time of entrance, 32 children were in care with 9 staff members present. The sample size consisted of 5 children?s records and 5 staff records. Children were observed eating, reading time and preparing for rest period.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Description: Based on record review and staff interviews, the licensee did not ensure all staff shall undergo a background check every five years to include a sworn statement, criminal records check (CRC) and search of the central registry check.

Evidence: Staff #4 did not have an updated CRC as the most recent was dated 9-5-2018.

Description: Based on record review and staff interviews, the licensee did not ensure each staff member had obtained by the end of the 30th day of hire the results of the Central Registry (CPS) finding for the state of Virginia.

Evidence: Staff # 1?s (hire date 7-24-2023) did not have documentation of CPS results. Staff #4 was observed working on the day of the inspection.
