Comments:
An unannounced monitoring inspection was conducted on 09/28/2023 from 10:30 AM to 12:40 PM. There were 39 children present, ranging in ages from three to five years old with eight staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and five staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-510-E
Description: Based on review of policy, the center?s procedures for administering medication did not include methods to prevent use of outdated medication.
Evidence
1. The ?Medication acceptance and administration? procedure provided by the center did not include a method to prevent use of dated medication.

Plan of Correction: A plan will be implemented to note the expiration date of medication on our office / school calendar (one month prior to expiration) as a reminder to director to advise parents get an updated medication. This will prevent any stored / locked medication to exceed the past date.

Standard #: 8VAC20-780-510-G
Description: Based on review of medication and interview, the center failed to ensure that medication is labeled with the child?s name, the name of the medication, the dosage and the time or times to be given.
Evidence:
1. An epinephrine pen was observed in a plastic bag with Child #6?s name on the bag. It was not labeled with the name of the medication, the dosage and the time or times to be given.
2. Staff #6 confirmed the label with the required information was missing.

Plan of Correction: The medication was returned to the parent on 9/28/23... the day of inspection. It was returned back to school with corrections made.

Standard #: 8VAC20-780-530-A-1
Description: Based on observations and interview, the center failed to ensure that at least one staff in each classroom or area where children are present have current certification in cardiopulmonary resuscitation (CPR).
Evidence:
1. Staff #5 was observed to be the only staff member on the playground with seven children.
2. Staff #5?s CPR certification expired on 09/15/2023. Staff #6 confirmed that Staff #5 did not have current CPR certification.

Plan of Correction: Staff member #5 that was 14 days expired is scheduled for a CPR / First Aide recertification class on Oct. 4, 2023.

Standard #: 8VAC20-780-530-A-2
Description: Based on observations and interview, the center failed to ensure that at least one staff in each classroom or area where children are present have current certification in first aid.
Evidence:
1. Staff #5 was observed to be the only staff member on the playground with seven children.
2. Staff #5?s first aid certification expired on 09/15/2023. Staff #6 confirmed that Staff #5 did not have current first aid certification.

Plan of Correction: 5 staff members have attended a CPR/ First Aide class on 10/5/23 to be certified.
