8VAC20-780 Staffing and Supervision

Comments:
A self-reported other monitoring inspection was initiated on 9/28/23 and concluded on 9/28/23. The inspector reviewed compliance in the area of staffing and supervision. The inspection started at 12pm and concluded at 1:15pm.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-340-A
Description: Based on documentation and interviews the center failed to ensure the care, protection, and guidance of a child.

Evidence:
Center staff were not aware that a two (2) year old child exited the classroom at approximately 4:15pm through the classroom door ,that leads outside, which opens onto the front outside area of the center. The child was found outside of the door in the grassy area, in front of the building, on 9/25/23.
The child was witnessed to be alone by a parent who arrived to pick up his child at approximately 4:15pm. A few minutes later (approximately 4:18pm), upon exiting with his child, the two (2) year old was still alone outside in the same area. This parent brought the child inside to the office at this time.
Staff realized the child was not visible within the classroom and were actively looking around the classroom when the child was brought into the center.
The child was outside alone for approximately three (3) minutes.

Plan of Correction: A meeting was held immediately after the incident with the three (3) staff who were in the classroom during the incident. A review of supervision requirements, classroom management, and the name to face documentation was conducted by center administration. All three (3) staff have received a write up and will be required to complete the on-line active supervision training.
The two doors in the classroom already had child door locks installed on the door handles, but these will be reviewed and replaced if deemed necessary to ensure this type of incident doesn't happen in the future.

Description: Based on documentation and interviews the center failed to ensure children are always within actual sight and sound supervision of staff.

Evidence:
Center staff did not ensure a two (2) year old child was in sight and sound supervision on 9/25/23 from approximately 4:15pm-4:18pm when the child exited the door within the classroom, that opens outside onto the front of the facility, and was found outside in the grassy area directly outside of the classroom door by a parent picking up his child.
Center staff stated that they did not know the child had exited the classroom until approximately 4:17pm when they didn't see him within the classroom and began looking in the classroom for him. Approximately a minute later the child was being brought into the center.
There were a total of 20 children with 3 staff in the classroom at the time of this incident.
