Comments:

An unannounced on-site renewal inspection was conducted on 10/02/2023. There were 25 children, ages 6 months-5 years, and 6 staff members present during the inspection. The inspector observed the following: free choice activities, lunch, and nap. A total of 5 children?s records and 5 staff records, were reviewed. The inspector discussed the following with staff: program leader qualifications and new hires. The inspector and the Licensing Administrator arrived at 11:45 AM and departed at 2 PM. Description:	Based on record review and interviews, the center failed to ensure that each child shall have a physical examination by or under the direction of a physician before the child?s attendance or within 30 days after the first day of attendance. Evidence: 1. The record for Child # 5 contained documentation that the first date of attendance for Child # 5 was 08/02/2022; there was no documentation of a physical exam on the day of the inspection.

Description:	Based on review of medications and authorizations, the center failed to ensure that all medications were returned to the parent or guardian within 14 days after the authorization expired or disposed of by the center. Evidence: Medication authorization for Child # 5 expired 14 days after 04/13/2023. As 10/02/2023, the medication had not been returned to the parent or disposed of by the center.
