Comments:
An unannounced monitoring inspection was completed on Thursday, October 5, 2023 at 2:55 pm until 4:38 pm at the center. There were 6 children present, ranging in ages from infancy to 3 years, with 4 staff supervising and 2 administrative staff at the facility. The inspectors completed observations of children in care in the classroom sitting at the table, completing handwashing, interacting with peers and staff, and eating snack. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, special services, background checks, and nutrition. A total of 4 child records and 5 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations:
Standard #: 8VAC20-780-70
Description: Based on review of staff records and interview, the center did not ensure to obtain documentation that two or more references were checked before employment or volunteering.

Evidence: In Staff#1's record (date of hire: 09/26/23) there was no signature of person making call found.
1. In Staff#2's record (date of hire: 08/15/23) there was no signature of person making call found.
2. In Staff#3's record (date of hire: 08/08/23) there was no signature of person making call found.
3. In Staff#4's record (date of hire: 08/15/23) there was no signature of person making call found.
4. In Staff#5's record (date of hire: 07/12/23) there was no documentation available for review for two or more references.
5. Interview with staff confirmed available documentation for references during the inspection.

Plan of Correction: The center director will update the forms and correct staff forms already completed.

Standard #: 8VAC20-780-280-B
Description: Based on observation, the center did not ensure all hazardous substances were kept in a locked place using a safe locking method that prevents access by children.

Evidence: During the inspection, the director's office door was opened and two containers of disinfectant spray and a cleaner with the labels "keep out of reach of children" and "caution" located on the bottom shelf accessible to the children in care.

Plan of Correction: The center director will ensure all hazardous substances are locked up and inaccessible to children. The director removed them immediately

Standard #: 8VAC20-780-570-A
Description: Based on observation and photographs, the center did not ensure when a child is placed in an infant seat or high chair, the protective belts shall be fastened securely.

Evidence: 1. In the infant room, Child#3 (age 14 months) was observed eating a snack in an infant seat with the protective belt missing. 2.Interview with staff confirmed the protective belt was missing for the seat in use by Child#3. 3. Photographs taken by the inspector during the inspection.

Plan of Correction: The center's director will double check chairs to ensure they have protective strapes and educate staff.
