Comments:
An unannounced monitoring inspection was conducted on 9/13/23 from 11:30 a.m. to 1:30 p.m. with the Professional Teacher and Program Manager. There were 28 children in care, ranging in age from two to five-years-old, supervised by five staff. The children were observed eating lunch and preparing for nap. Five child records and five staff records were reviewed. Three medications and authorization forms were reviewed and the center had two staff current in Medication Administration Training (MAT). Required postings were observed. The attendance, menu, and emergency drill log were reviewed. The first aid kit, flashlight, and battery operated radio were observed. Violations:
Standard #: 8VAC20-780-270-A
Description: Based on observation, the center did not ensure all areas and equipment of the center were maintained in a safe condition.

Evidence: In the Pre-K class, peeling paint was observed in the block area.

Plan of Correction: Peeling paint will be removed and surface will be repainted

Standard #: 8VAC20-780-330-B
Description: Based on observation and measurements, it was determined that the slides on the playground did not have the required amount of resilient surfacing underneath and surrounding the structure.

Evidence: At the base of slide #1, 47 inches away from the fall zone of the slide exit , there was one and three quarter inches of pea gravel. At the base of slide #2, 58 inches away from the fall zone of the slide exit, and surrounding the composite play structure there was less than four inches of pea gravel. The requirement is six inches of pea gravel.

Plan of Correction: The pea gravel will be raked to fill the requirement of six inches at the base slide #! and slide #2. Safety mats will also be placed at the base of all slides. Pea gravel has been raked immediately and will be maintained regularly. Safety mats will be delivered no later than October 26th.

Standard #: 8VAC20-780-500-A
Description: Based on observation, hand washing procedures after toileting were not always followed.

Evidence: Licensing Inspector (LI) observed a teacher returning a child to the group after changing a soiled diaper without washing the child?s hand or their own hands with soap and running water.

Plan of Correction: Staff will be trained on the diaper changing procedures per licensing standard 8VAC20-780-500 A

Standard #: 8VAC20-780-500-B
Description: Based on observation, the diapering surface was not always being cleaned with soap and water and sanitized after each use.

Evidence: In the two year classroom, the LI observed a teacher clean the diaper surface with soap and water only, no sanitizer was used. The requirement is the surface shall be cleaned with soap and water and sanitized after each use

Plan of Correction: Staff will be trained on the handwashing procedure per licensing standard 8VAC20-780-500B.

Standard #: 8VAC20-780-560-G
Description: Based on observation, the center did not ensure that when food is brought from home that it is labeled in a way that identifies the owner.

Evidence: In the Pre-K classroom there were three lunch boxes with no name

Plan of Correction: Lunch boxes will be labeled immediately at drop off with children's name and date.
