Comments:

An unannounced monitoring inspection was conducted with the director on September 21, 2023, beginning at 6:58am and ending at 7:50am. There were 30 school-aged children in the care of 4 direct care staff. The children were observed doing an art project and playing games. Five children?s records were reviewed and in compliance. Four staff records were reviewed and in compliance. The first aid kit, flashlight, and battery-operated radio were observed and in compliance. Five medications were reviewed. The daily attendance and emergency drill log were reviewed. The most recent evacuation drill was documented as 9/19/2023. The most recent shelter-in-place drill was documented as 9/4/2023. Standard #:	22.1-289.011-F
Description:	Based on observation, the center failed to post the license and any other documents required by the Superintendent in a conspicuous place on the licensed premises. Evidence: The most recent inspection was located on the parent board but had a coversheet obscuring it from view.
Plan of Correction:	The coversheet was removed from in front of the inspection summary during the inspection.
Standard #:	22.1-289.035-A
Description:	Based on the review of three staff records, not all of the facility staff had documentation of the required subsequent background checks. Evidence: Staff 1's most recent fingerprint documentation was dated 4/18/2018. The most central registry findings were dated 3/7/2017.
Plan of Correction:	The facility staff will provide documentation of updated fingerprinting and central registry background checks within 10 days.
Standard #:	8VAC20-780-130-A
Description:	Based on a review of five children's records, the facility failed to obtain documentation of immunization for all children enrolled prior to their first day of attendance. Evidence: Child A's record was missing documentation of an immunization. Their first day of attendance was August 21, 2023.
Plan of Correction:	The facility staff will request the missing documentation from the parent and place it in the child's record upon receipt.

Description:	Based on a review of five children's records, not all of the children had documentation of a physical examination within 30 days after their first day of attendance. Evidence: Child A's records was missing documentation of physical examination. Their first day of attendance was 8/21/2023.
