An unannounced monitoring inspection was conducted on 10/4/2023 from 10:00 am- 11:35 am. There were 36 children (infants-5yrs.) supervised by 12 direct care staff. The physical plant, 3 staff records, 5 children?s records, medications, evacuation drills, and policies were inspected. Children were observed participating in group play, reading books, and playing outside. Areas of non-compliance are identified in the Violation Notice. Standard #: 8VAC20-780-40-M Description: Based on observation and staff interviews, the center did not maintain a current written list of all children's allergies, sensitivities, and dietary restrictions, dated and confidential in each room.
Evidence:
1. In the infant room, the allergy list was not kept confidential.

Standard #: 8VAC20-780-270-A Description: Based on observation and staff interviews, areas and equipment of the center, inside and outside, were not maintained in a clean, safe and operable condition.
