Comments:
An unannounced renewal inspection was conducted on 10/02/2023 from 12:30pm to 4:10pm. At the time of the inspection there were 12 children in care and 3 staff members present. The record sample size consisted of 5 children?s records and 5 staff records. The inspector toured the facility indoors and outdoors and observed classroom activities. Children were observed napping, and engaged in floor play. Emergency supplies, evacuation drills and required postings were reviewed during the inspection. The information gathered during the inspection determined non-compliance with applicable standards or law and were documented on the violation notice and discussed during the exit interview with the Program Director.

Violations:
Standard #: 8VAC20-780-140-A
Description: Based on a record review and interview, it was determined that the facility did not ensure that all children had documentation of a physical examination by or under the direction of a physician.
Evidence:
Child #4 (enrolled 1/18/23) did not have documentation of having received a physical. The Program Director confirmed the child?s record did not contain this information.

Standard #: 8VAC20-780-270-A
Description: Based on observation and interviews, it was determined that the facility did not ensure that all areas and equipment of the center shall be maintained in a safe and operable condition.
Evidence:
Eight of the steel spikes anchoring the border around the large playground were lifting from the inset of the border. The spikes were above the inset by approximately 1? to 3? high creating a tripping hazard.

Description: Based on record review and interviews, it was determined the facility did not ensure that diaper ointment shall be kept in a manner that is inaccessible to children.
Evidence:
1.In the toddler/preschool classroom, diaper ointment was stored in the lower cabinet of a changing table. The diaper changing cabinet was unlocked and within reach of children.

Description: Based on observation and interviews, it was determined that the facility did not ensure that all of the required nonmedical emergency supplies were available.
Evidence:
1. The center did not have a working, battery-operated radio.
2. Staff #5 (Program Director) confirmed the center did not have a working battery operated radio.

Description: Based on observation and interviews, it was determined the facility did not ensure that when food is brought from home it is clearly dated and label in a way that identifies the owner
Evidence:
1.In the refrigerator in the infant room there were 3 bottles of formula for children in care that were not dated.
2.Staff #2 confirmed the bottles were not dated.
