Comments:
A monitoring inspection was initiated on 10/05/2023 and concluded on 10/05/2023. There were 18 children present, ranging in ages from 2 years to 5 years, with 6 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 4 staff records were reviewed.

Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 8VAC20-780-60-A
Description: Based on a review of children?s records, the center failed to ensure that each child's record contained the required information.

Evidence:
1. The record of Child 3, did not contain documentation of the following information: the child's first date of attendance.
4. The record of Child 4, did not contain documentation of the following information: the child's first date of attendance.
5. The record of Child 5, did not contain documentation of the following information: the child's first date of attendance.

Standard #: 8VAC20-780-280-B
Description: Based on observation and interview, the center failed to ensure that hazardous substances were kept in a locked place using a safe locking method that prevents access by children.

Evidence:
1. In the Moonbeam Room (Toddler Rm), there was a broken lock in the upper bathroom cabinet containing BioPure Wipes and Oxvir Cleanser. The label on the BioPure Wipes stated "flammable" and "keep out of reach of children". The label on the Oxvir cleanser stated "caution" and "keep out of reach of children".
2. Also, in the Moonbeam Room (Toddler Rm), there was a broken lock in the classroom cabinet near the dishwasher containing Bleach water and Dishwasher Pods. The Bleach water states "Caution" and "Keep out of reach of children". The Dishwasher Pods stated "Caution" and "Keep out of reach of children".

Description: Based on record review, the center failed to ensure that lockdown procedures shall be practiced at least annually.
