An unannounced inspection was initiated on August 23, 2023 in response to complaints received on August 7, 2023, August 8, 2023, and August 10, 2023 regarding allegations in the area of forbidden actions. The inspector and other local agencies conducted interviews with center staff and observations were made in the center on August 23, 2023.
The inspector completed additional interviews, observations, and reviewed documentation on August 24, 2023. The inspector was onsite from approximately 9:00 am to 10:30 am on August 24, 2023.
The preponderance of the evidence gathered during the investigation supports noncompliance in the area of forbidden actions. The complaint is determined to be valid.
Please complete the plan of correction and date to be corrected for each violation cited on the violation notice and return it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word corrected is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Standard #: 8VAC20-780-410-1 Complaint related: Yes Description: Based on interviews with 5 staff, the center failed to ensure roughly handling of a child was forbidden.
Evidence:
1. During an interview with Staff 1, it was acknowledged that she overreacted and grabbed Child 1 by the neck area of his shirt and heard a ?little thump? when she grabbed Child 1.
2. Staff 1 acknowledged, she had a lot of personal things going on that day and although she was attempting to avoid Child 1 from grabbing hot coffee she had in the classroom, she could have reacted more calmly.Plan of Correction: Not available online. Contact Inspector for more information.
