Comments:
An unannounced monitoring inspection was conducted on 10/2/2023 from 12:57 pm to approximately 3:20 pm. There were 18 children in care, ages seven months to four years, in three groups under the care of two to four (it varied) staff. The Licensing Inspector reviewed four children?s records, four staff records, and other program records. The children were observed napping and getting up.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Description: Based on record review, the center failed to obtain a fingerprint eligibility letter for a new staff person prior to hire.

EVIDENCE:
The record for Staff 4 shows the most recent documented hire date is 12/5/2022. The fingerprint eligibility letter is dated 5/24/2023.

Description: Based on record review and interview, the center failed to maintain a written record of daily attendance that documents the arrival and departure of each child in case as it occurs for each group of children.

EVIDENCE:
1. There were seven children in the 3- and 4-year-old group, and the written record of attendance showed there were six children.

2. There were seven children in the 7 months to 17 months old group, and the electronic attendance showed there were six children. Staff one stated that one parent did not sign in electronically because her phone had a problem.

Description: REPEAT VIOLATION

Based on observation, the center failed ensure that areas and equipment of the center, inside and outside, was maintained in a clean, safe, and operable condition.

EVIDENCE:
1. Four gray and red tricycle seats were cracked at the rear of the seat causing a pinch point. There were no children observed using the tricycles, but they were readily accessible to children when playing outside.

2. The blue and yellow slide structure has an exposed anchor approximately one inch above the ground on the right side directly behind the slide portion. There is a rusted section with peeling paint behind a seat on the climbing section of the equipment. There were no children observed using the equipment, but it was readily accessible to children when playing outside.

Description: Based on observation, the center failed to maintain the staff to child ratio for the birth up to 16 months age group.

EVIDENCE:
At the start of the inspection, the birth up to 16 months age group had one staff person in the group and seven children in the group. The required ratio is one staff person to four children for this age group.
