Comments:
An unannounced monitoring inspection was conducted on 10/6/2023 from 9:10am to 1:00pm. There were 61 children ages 3 years old to age 6 years old supervised by 8 staff. The physical plant, outdoor playground area, programming, attendance, menus, 5 staff records, 5 children?s records, 4 medications with corresponding authorization records, emergency drills, injury reports, emergency supplies, and policies were reviewed. Children were observed participating in math activities, literacy activities, writing exercises, and fine motor activities. Handwashing procedures were also observed. There was an adequate number of staff present with current certification in Medication Administration Training (MAT), CPR and First Aid, and Daily Health Observation (DHO) training. Areas of non-compliance are identified in the Violation Notice.

Standard #: 8VAC20-780-510-E
Description: Based on review of 4 medications and medication authorizations, the center did not ensure they follow the policy for administration of medication.

Evidence:
For the medications for Child #5 and Child #6, the duration of the parent?s authorization was not renewed after 10 workdays nor was written medication authorization given from the child?s physician and parent for a long term medication.

Plan of Correction: Corrected violation for Child #5 and #6 by the end of the day. Parents filled out an authorization form for the next 10 work days, until parents obtain the physician long term medication authorization. If we have not received the physician form within the 10 working days, parents were advised to fill out another 10 day authorization form.
