A monitoring inspection was conducted on 10/11/2023 between the hours of approximately 8:00 am and 9:00 am. There were 17 children present, with 2 staff supervising. Children were observed while playing board games, doing crafts, coloring, playing games on the computer and reading books.
The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. There were no medication on site today. A total of 3 child records and 2 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #: 8VAC20-780-130-A Description: Based on record review, the program did not ensure to obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.
Evidence:
1) The record for Child #3 (start date 08/23/2022) did not contain documentation of immunizations.Plan of Correction: Attained records from parent.
