Technical Assistance:
Recommend reviewing all children's records to ensure the parents have included their work phone number and place of employment.

Comments:
An unannounced monitoring inspection was conducted on October 11, 2023 between the hours of 10:45am and 1:15pm. There was a total of 39 children with 8 staff members supervising. Children were observed engaged in group activities, story time, music, playing with age appropriate toys and playing on the playground. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, children and staff records, special care and emergencies and nutrition. The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 8VAC20-780-60-A
Description: Based on record review and discussion with director, the facility failed to ensure that the separate record for each child enrolled shall contain all the elements as required by the standards.
Evidence:
1. The record for Child #2 did not have the work phone number and place of employment of each parent who has custody.
2. The record for Child #3 did not have the work phone number and place of employment of each parent who has custody.
3. The record for Child #4 did not have the work phone number and place of employment of each parent who has custody.
4. The record for Child #5 did not have the work phone number and place of employment of each parent who has custody.

Plan of Correction: We will contact the parents today and have them add the information to the record.

Standard #: 8VAC20-780-80-A
Description: Based on record review and discussion with the director, the center did not maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.
Evidence:
1. Child #5 was not signed in on the daily attendance at the time of the inspection.
2. The director stated she greeted Child #5 upon arrival to school (approximately 8:30am).
Repeat Violation

Plan of Correction: I will remind the teachers to confirm every child is signed in before giving the record to the office.

The violation was corrected during the inspection.

Standard #: 8VAC20-780-550-D
Description: Based on record review and interview, the center did not implement a monthly practice evacuation drill.
Evidence:
1. The evacuation drill log did not have documentation of a practice drill for June and July.
2. The director acknowledged the center did not practice an evacuation drill for the months of June and July.

Plan of Correction: I will ensure we practice the drills each month.
