Technical Assistance:

Locked storage of staff handbags was discussed with the program director. The required one hour of daily outdoor play was discussed with staff and the program director.

Comments:

A monitoring inspection was conducted on 10/12/2023. The inspector arrived at 10:20 AM and departed at 1:10 PM. There were 14 children in care ranging in age from infant - four years with 5 staff.The inspection included the areas of administration, physical plant, staffing/supervision, programming, special care and emergencies and nutrition. A total of 3 child records and 6 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program reviewed with the program director during the exit interview.

Description:	Based upon a review of children's records, the facility has not ensured that each child's record includes the names, addresses and telephone numbers for two persons to be contacted in an emergency if a parent cannot be reached. Evidence: The record provided for child 3 did not include the address for one of the two emergency contact persons.

Description:	Repeat Based upon review of staff records and staff interview, the facility has not ensured that each staff record includes all information. Evidence: 1. The records provided for staff 2, 3 4, and 6 did not include the dates of hire or the positions for which the staff are hired. 2. The records provided for staff 2, 3, 4 and 5 did not include documentation to demonstrate that staff 2, 3, 4 and 5 received the orientation training required. 3. Staff 1 verified that staff 2, 3 and 4 were hired as teachers (program leaders). The records did not include documentation that the individuals possess education, certification and/or experience required by the job position. 4. The records provided for staff 2 and 3 did not included documentation that at least two references as to the individuals' character, reputation and competency were checked before employment.. 5. Description:	Based upon observation, the facility has not ensured that hazardous substances are kept in a locked place using a safe locking method that prevents access by children. Evidence: Repeat There was a bottle of antibacterial hand soap labeled "warning" and "Keep out of reach of children" in an unlocked tall cabinet n the bathroom used by the children at the end of the hallway.

Description:	Based upon observation, the facility has not ensured that electrical outlets have protective covers. Evidence: An electrical outlet in the hallway connecting all the classrooms was not capped.

Description:	Based upon observation, the facility has not ensured that diapering changing areas are equipped with warm running water and that diapers are disposed in a container where a staff person's hands nor the soiled diaper touch the outside of the containing when disposing dirty diapers. Evidence: 1. The portable hand washing station in the 12 month old and toddler classroom did not have warm water. 2. Staff 4, in the twelve month old/toddler classroom was observed opening the lid of the diaper disposal system with her hands when disposing of a soiled diaper.
