Comments:
An unannounced monitoring inspection was conducted today.
All groups were in ratio during the inspection. Children were observed resting quietly in all rooms.
2 staff and 3 child records were reviewed.
Medications, emergency evacuations, diapering, cot spacing and top and bottom covers for cots were all in compliance today.
Violations were reviewed with the Head of School. See Violation Notice for non-compliances cited today.

Description: Based on observation, the most recent inspection report was not posted.
Evidence:
The most recent inspection report (8/10/23) was not posted to the public. A statement that the report was available upon request was posted.

Plan of Correction: The last report was posted during the inspection. Moving forward the reports will be posted as they are received.

Standard #: 8VAC20-780-240-I
Description: Based on a review of documentation, orientation documentation does not contain all required information.
Evidence:
The orientation form in use does not capture the Method of delivery for training.

Plan of Correction: Orientation form was updated 9/21/23 and contains all DOE required components. Onboarding for staff member in question was completed prior to the form update; however, method of delivery was in person and compliant with DOE requirements.

Standard #: 8VAC20-780-370-5
Description: Based on a review of documentation, infants that cannot turn themselves over, have not spent 30 minutes each day to facilitate upper body strength and misshapen heads.
Evidence:
Infant #1's documentation of tummy time showed:
*15 minutes spent on their stomach on 10/10/23.
*10 minutes spent on their stomach on 10/11/23.

Plan of Correction: Guides in the Infant Program and HOS discussed what had been happening in the program regarding Tummy Time. A plan has since been put in place to ensure that 30 minutes of Tummy Time, spread out over tolerable increments for each infant, will now be incorporated into every infants daily routine. Moving forward, HOS will routinely monitor/review plan and supporting documentation.

Standard #: 8VAC20-780-420-E-1
Description: Based on staff statement and a review of documentation, the center did not record on the infant daily sheet, the amount of time an infant spent on their stomach.
Evidence:
The amount of time infants that cannot turn themselves over is not included as part of the daily record provided to parents. It is only recorded in the classroom notebook.

Plan of Correction: Tummy Time had been documented in a paper log kept in the center with photos of the activity having been sent to parents. Moving forward, record of cumulative minutes will be kept in paper log as well as notes section within TP, which is shared with parents daily.

Standard #: 8VAC20-780-550-P
Description: Based on a review of injury reports, all required components were not documented.
Evidence:
5 of 7 reports reviewed were missing the time and date of parent notification.

Plan of Correction: Staff were reminded to complete all components of the injury report at a staff meeting on 10/12/23.
