Standard #: 8VAC20-780-160-C Description: Based on record review and interview it was determined the center did not ensure that staff obtained and submit the results of a follow-up tuberculosis screening at least every two years from the date of the first initial screening or testing.
Evidence:
1. The record for staff #1(hired 5/16/22) did not contain documentation of an updated TB test. The test on file was dated 4/22/21.
2. Staff confirmed a more recent TB test/screen was not available during this inspection.Plan of Correction: Staff will get an updated TB test/screening.
Standard #: 8VAC20-780-280-B Description: Based on observation and interview it was determined the center did not ensure that all hazardous substances were kept in a locked place using a safe locking method that prevents access by children.
Evidence:
1. There were unlocked cleaning solutions in unlocked cabinets in the green and blue classrooms.
2. Staff confirmed these cabinets were unlocked during this inspection.Plan of Correction: Center staff will make sure all chemicals are locked.
