A monitoring inspection was conducted on 10/16/2023. There were a total of 86 children with 21 staff members present. Children were observed during outside play, napping, and engaged in small group centers. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today in order for it to be included on the website. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Standard #: 8VAC20-780-160-A Description: Based on a review of staff records, the director did not ensure each staff member submitted documentation of a negative tuberculosis screening completed within the last 30 days.
Evidence:
Staff #2 (date of hire 8/8/2023) had a TB test dated 7/1/2023.
Staff #3 (date of hire 8/8/2023) had a TB test dated 6/27/2023.Plan of Correction: Ensure Tb tests are done no earlier than 30 days prior the first day.
Standard #: 8VAC20-780-500-B Description: Based on observation, the director did not ensure the diapering surface was cleaned with soap and at least room temperature water and sanitized after each use.
Evidence:
The diapering station in room 105 was not cleaned and sanitized after a baby had a diaper change.Plan of Correction: Re-training infants and toddler teachers on proper changing diaper steps and techniques.
