An unannounced monitoring inspection was conducted with the director on October 16, 2023 beginning at 4:30 and ending at 5:15pm. There were 18 school-aged children in the direct care of three staff members. The children were observed playing with toys and games during the inspection. The children were also observed playing outside. Six children?s records were reviewed. Two staff records were reviewed and in compliance. Four medications were reviewed and in compliance. Facility policies and procedures were reviewed and in compliance. The first aid kit, flashlight, and battery-operated radio were observed and in compliance. The menu, daily attendance, and emergency drill log were reviewed. The most recent evacuation drill was documented as 9/29/23. The most recent shelter-in-place drill was documented as 5/23/23. The health and fire inspection were reviewed and in compliance.

Standard #: 8VAC20-780-130-A Description: Based on a review of six children's records, the facility failed to obtain immunizations for all children enrolled before the their first day of attendance. Evidence: Child A, child B, and child c were missing documentation of immunizations in their records. Plan of Correction: The director stated she would request the missing documentation from the parents.
Standard #: 8VAC20-780-140-A Description: Based on a review of six children?s records, the facility failed to obtain documentation of a physical examination for all children within 30 days of their first day of attendance. Child A, child b, and child c were missing documentation of physical examinations. Plan of Correction: The director stated she would request the missing documentation from their parents.
