Technical Assistance:
Discussed with the provider: the center must have written procedures for prevention of abusive head trauma.

Discussed with the provider: the next anticipated inspection will be the provider's renewal. Provider was asked to submit all renewal paperwork to the licensing office via the online portal 45 days prior to expiration of the license. The provider was notified that the new renewal application is available on the VDOE website.

Comments:
An unannounced Monitoring Inspection took place on October 12, 2023 between the hours of approximately 11:20 a.m. and 1:10 p.m. There were 4 classrooms observed with a total of 40 children with 5 staff within the supervision guidelines. The children were observed playing outside, reading a book, working a sensory activity and other organized activities. Positive interaction between staff and children were observed. A complete inspection of the physical plant, children and staff records, fire drill log, medication and emergency procedures was conducted during this inspection. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #: 8VAC20-780-60-A-8
Description: Based on record review, the center failed to obtain a written care plan for each child with a diagnosed food allergy.

Evidence:
1. Child #4?s record indicated an allergy to nuts.
2. Child #4?s record did not contain a written care plan.

Description: REPEAT VIOLATION

Based on observation, hazardous substances were not kept in a locked place.

Evidence: One spray bottle labeled Lysol, one air freshener, and two disinfectant sprays were located in an unlocked cabinet in the toddler classroom.

Plan of Correction: Will lock the cabinet

Description: Based on observation, the center failed to meet all requirements when diaper ointment or cream is used.

Evidence:
1. An ointment belonging to Child #4 was not labeled with the child?s name.
2. An ointment was located under the changing table in the toddler classroom, accessible to children.
3. A diaper ointment was in a bin at the bottom of the cubbies in the toddler classroom, accessible to children.

Description: Based on observation and interview, the center failed to meet all requirements when food is brought from home.

Evidence:
1. A food container belonging to Child #1 was not dated and labeled in a way that identifies the owner.
2. There were two juice boxes, one bag of chips, two boxes of graham snacks, seven packs of crackers and eight applesauce pouches not labeled in a way that identifies the owner. Staff #4 confirmed that the food was brought from home for Child #1.
