Standard #: 22.1-289.035-A Description: Based on a review of staff files, the facility failed to ensure that all repeat fingerprint background checks are conducted every five years. Evidence: 1. There is no repeat fingerprint background check on file for Staff #1. The last fingerprint on file is dated 10-4-18 Plan of Correction: The program director reported that a repeat background check will be conducted.
Standard #: 8VAC20-780-340-C Description: Based on observation, the facility failed to ensure that one staff member and an immediately available staff member, volunteer or other employee who is at least 16 years of age are present when children are in care.
Evidence:
