An unannounced monitoring inspection was conducted on-site October 5, 2023. The director was available during the inspection. There were 45 children present with 14 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 6 child records and 5 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Standard #: 8VAC20-780-280-B Description: Based on observation, the center failed to ensure that hazardous substances, such as cleaning materials, were kept in a locked place using a safe locking method that prevents access by children.
Evidence: 1. In the two year old classroom there was an unlocked cabinet that contained three cans of shaving cream with labels that state "keep out of the reach of children" and "caution." There was a can of spray paint in another unlocked cabinet with a label that states "keep out of reach of children" and "danger."
2. In the preschool classroom there was an unlocked cabinet under the sink within reach of the children that contained disinfectant and Mr. Clean cleanser with labels that stated "keep out of the reach of children" and "caution."Plan of Correction: Not available online. Standard #: 8VAC20-780-500-A Description: Based on observation, the center failed to ensure that staff and children's hands shall be washed with soap and running water after a diaper change.
Evidence: Staff #2 was observed changing a diaper in the two year old classroom. After the diaper change staff #2 did not wash her hands and did not wash the child's hands.Plan of Correction: Not available online. Contact Inspector for more information.
Standard #: 8VAC20-780-500-B Description: Based on observation and interview, the center failed to ensure a nonabsorbent surface for diaper or changing shall be used. For children younger than three years, this surface shall be a changing table or countertop designated for changing.
