Provided consultation on the following:
8VAC20-780-500B Pull ups should be placed in foot operated trash cans where a staff member does not need to touch the outside of the can with the diaper or their hand.
8VAC20-780-520C Diaper creams should be stored in an area inaccessible to children.
22.1-289.011 The last inspection should be posted in a conspicuous place at the center.

Comments:
An unannounced monitoring inspection was conducted on 10/16/2023 from 9:40am to 12:55pm. There were 93 children ages 4 months to age 5 years old supervised by 18 staff. The physical plant, outdoor playground area, programming, attendance, menus, buses, 5 staff records, 5 children?s records, 5 medications with corresponding authorization records, emergency drills, injury reports, emergency supplies, and policies were reviewed. Children were observed participating in snack, nap, group play, reading, transitions, singing, computer lab, music and outside play. Handwashing procedures were also observed. There was an adequate number of staff present with current certification in Medication Administration Training (MAT), CPR and First Aid, and Daily Health Observation (DHO) training. Areas of non-compliance are identified in the Violation Notice.

Standard #: 22.1-289.035-A
Description: (Repeat Violation) Based on review of 5 staff records, the Center did not obtain a sworn disclosure statement, Central Registry check and Criminal History check every five years from the date of the last background checks.

Evidence:
1)The record for Staff #3 did not contain a Central Registry check every five years from the last check. The background check was dated 9/7/17.
2)The record for Staff #4 did not contain a Criminal History check or Sworn Disclosure statement every five years from the last check. The last Criminal History check was dated 8/21/18 and the last Sworn Disclosure statement was dated 8/20/18.

Plan of Correction: Review all staff files and ensure they are in compliance.

Standard #: 8VAC20-780-160-A
Description: (Repeat Violation) Based on review of 5 staff records, the center did not obtain documentation of a negative tuberculosis (TB) test or screening for staff at the time of employment and prior to contact with children and within 30 days prior to employment.

Evidence:
1)The file for Staff #1 (date of hire 2/6/23) did not contain documentation of a negative tuberculosis (TB) test or screening.
2)The file for Staff #5 (date of hire 2/6/23) did not contain documentation of a negative TB test or screen in within 30 days prior to employment. The TB screening on file was dated 9/23/22.

Plan of Correction: Review all staff files and ensure each is in compliance.

Standard #: 8VAC20-780-160-C
Description: Based on review of 5 staff records, the center did not obtain documentation of a negative tuberculosis (TB) test or screening for staff every 2 years from the date of the initial screening or testing.

Evidence:
1)The record for Staff #3 did not have documentation of a negative TB test or screening within 2 years of the last TB test. The most recent TB test on file was dated 1/2/21.
2) The record for Staff #4 did not have documentation of a negative TB test or screening within 2 years of the last TB test. The most recent TB test on file was dated 8/23/18.

Plan of Correction: Review all staff files and ensure each is in compliance.

Standard #: 8VAC20-780-80-A
Description: (Repeat Violation) Based on review of the written attendance record and interview with staff, the center did not ensure they maintained a written record of daily attendance for each group of children documenting the arrival and departure of each child in care as it occurs.


Evidence: During inspection in the Gard 2 CD classroom (age 2), there were 16 children present, but the written record reflected there were 21 children present.

Plan of Correction: Retrained teachers in the Gard 2 classroom.

Standard #: 8VAC20-780-330-B
Description: (Repeat Violation) Based on observation and inspection of the playground, the licensee did not to ensure that the required resilient surfacing surrounding playground equipment with moving parts or climbing apparatus created a sufficient fall zone.


Evidence: The Obstacle course on the playground has an above ground tunnel that is 18 inches off the ground with 1 inch of wood chips on the entry point side that faces the building instead of the required 6 inches.

Plan of Correction: Schedule additional mulch to be delivered.

Standard #: 8VAC20-780-570-E
Description: (Repeat Violation) Based on observation, the center did not ensure that prepared infant formula was dated and labeled with the child?s name.

Evidence: In the Infant B classroom, there were 2 children who had bottles stored in the refrigerator and not dated.

Plan of Correction: E.D. will check every bottle as it is brought in each day.

Standard #: 8VAC20-780-580-B
Description: Based on review of documentation and interview with staff, the center did not ensure that vehicles used to transport children met the safety standards set by the Department of Motor Vehicles to assure the safety of the children.

Evidence:
1)The safety inspection and registration for bus ending in license plate #4308 expired on 08/31/2023 and the center has been using the bus to transport children.
2)The safety inspection for bus ending in license plate #5313 expired on 08/31/2023 and the registration for this bus expired on 3/31/2023 and the center has been using the bus to transport children.

Plan of Correction: Have registration on each bus renewed.
