8VAC20-780-420E3 Parents should at least annually provide confirmation that the required information in the child's record is up to date.

Comments:
An unannounced monitoring inspection was conducted on 10/18/2023 from 10:00am to 11:35am. There were 40 children ages 16 months to age 5 years old supervised by 9 staff. The physical plant, outdoor playground area, programming, attendance, menus, 4 staff records, 3 children?s records, 2 medications with corresponding authorization records, emergency drills, injury reports, and policies were reviewed. Children were observed participating in reading, table toys, coloring, math and outside play. Handwashing procedures were also observed. There was an adequate number of staff present with current certification in Medication Administration Training (MAT), CPR and First Aid, and Daily Health Observation (DHO) training. Areas of non-compliance are identified in the Violation Notice.

Standard #: 8VAC20-780-60-A
Description: Based on review of 5 child records, the Center did not ensure they obtain all of the required documentation for child records.

Evidence:
1)The record for Child #3 did not contain authorization for emergency medical care should an emergency occur when the parent cannot be located immediately.
2)The record for Child #2 did not contain a written care plan for a diagnosed food allergy, to include
instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.

Standard #: 8VAC20-780-80-A
Description: (Repeat Violation)Based on review of the written attendance record and interview with staff, the center did not ensure they maintained a written record of daily attendance for each group of children documenting the arrival and departure of each child in care as it occurs.


Evidence: During inspection in the Preschool classroom, there were 13 children present, but the written record reflected there were 12 children present.

Description: (Repeat Violation)Based on observation, the center did not ensure that hazardous substances were kept in a locked place using a safe locking method that prevents access by children.

Evidence:
1)During inspection in the Preschool classroom, there were 12 tubes of toothpaste that were not stored in a locked area.
2) During inspection in the Pre-Kindergarten classroom, there were 13 tubes of toothpaste that were not stored in a locked area.

Description: (Repeat Violation)Based on observation, the center did not ensure that the diapering surface should only be used for diapering or cleaning children.
Evidence: In the Toddler classroom on the diapering surface a cot with sheets had been placed there for storage.

Description: (Repeat Violation) Based on observation, the center did not ensure that food brought from home was clearly dated and labeled in a way that identifies the owner.

Evidence: In the Preschool classroom, there was a bag of chips and a bag of gummy snacks that had been brought from home that were not labeled with a child?s name or date.
