Comments:
An unannounced monitoring inspection was conducted on October 18, 2023, from 10:00am-12:20pm. At the time of entrance, 27 children were in care with 9 supervising staff members present. The inspector reviewed five children's records and five staff records. Children were observed during snack time, handwashing, bottle feeding, engaged in group activities and outdoor playground time. Areas of non-compliance are identified on the violation notice. Repeat violations were found and are identified within this report.

Please complete the "plan of correction" an "date to be corrected" for each violation cited on the violation notice and return to the department within 5 business days from receipt. Please specify how the deficient practice will be or has been corrected. Please do not use staff names; list staff by positions only.

Any plans of correction not received within 5 business days will result in the plans not appearing on the Violation Notice and public website.

Violations:
Standard #: 22.1-289.035-B-4
Description: Based on record review, the child day center did not obtain a sex offender registry check and a search of the child abuse and neglect registry of an individual that has resided outside of the state of Virginia in the preceding five years.
Evidence:
1. Staff #2's record (Date of hire-09/11/2023) has documentation that states that they have lived in the state of Maryland in the preceding five years. Staff #2's record did not contain documentation of an out of state sex offender registry search prior to the first day of employment. Staff #2 record did not contain documentation of an out of state child abuse and neglect search request by the end of the of the 30th day of employment.

Plan of Correction: Submit form to MD for sex offender registry / abuse & neglect. Ensure new hires don?t have out of state addresses in which qualifies them to need additional backgrounds.

Standard #: 8VAC20-780-60-A
Description: Based on record review, children's records did not contain required information.
Evidence:
1. Child #3's (Start date-10/11/2022) and Child #5's record(Start date-05/23/2022) did not contain documentation of child updates and confirmation of up to date information in the child's record.

Plan of Correction: Ensure all families update enrollment information verbally and via signature in packet.

Description: Repeat Violation

Based on record review, required records were not kept for each staff person.
Evidence:
1. Staff #2's record (Date of hire-09/11/2023) contained documentation of two references as to character and reputation as well as competency were not checked before employment. Documentation of two reference checks were conducted on 09/12/2023.
2. Staff #4's record (Date of hire-08/16/2023) contained documentation of two references as to character and reputation as well as competency were not checked before employment. Documentation of two reference checks were conducted on 08/16/2023.

Plan of Correction: Ensure the people on the references get back to me so I can fill out the reference paper before start date.

Standard #: 8VAC20-780-240-A
Description: Based on record review, the Virginia Department of Education-sponsored orientation course was not completed within 90 calendar days of employment.
Evidence:
1. Staff #1's record (Date of hire11/14/2022) contained documentation of the Virginia Department of Education-sponsored orientation course completed on 05/26/2023.

Plan of Correction: Ensure all employees complete 10hr training in 90 days, regardless of degree status.

Standard #: 8VAC20-780-280-B
Description: Repeat Violation

Based on observation, hazardous substances as cleaning materials were not kept in a locked place using a safe locking method that prevents access by children.
Evidence:
1. In the Two's classroom, a bottle of antibacterial cleaner was located in a high cabinet without a lock.

Plan of Correction: Ensure hazardous materials are locked up at all times.

Standard #: 8VAC20-780-500-A
Description: Based on observation, staff did not wash their hands with soap and running water after contact with body fluids.
Evidence:
1. In the Toddler's classroom, Staff #4 was observed wiping a child's nose and not washing their hands after coming in contact with body fluids.

Plan of Correction: Go over hand washing tactics and when applicable.
