Today an unannounced renewal Inspection took place between the hours of 9:45am and 11:45am. At the start of the inspection, there were 2 classrooms observed with a total of 11 children with 2 staff outside of the supervision guidelines; 2 additional staff arrived during the inspection. The children were observed playing freely, coloring and eating a morning snack. A complete inspection of the physical plant, children and staff records, fire drill log and emergency procedures were observed during this inspection. Center management stated that there are no medications kept on site. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Areas of non-compliance are identified on the Violation Notice.

Based on record review, childrens records did not contain all required information.
Evidence:

3. Child #4s record does not contain a second emergency contact or documentation that the child's age and identity has been verified.

Standard #: 8VAC20-780-70 Description: Based on record review, staff records did not contain all required information.
Evidence:
Staff #2, staff #3 and staff #4s records do not contain the name, address, and telephone number of a person to be notified in an emergency and verification of education.Plan of Correction: Staff #4 is no longer employed. I'll get emergency contacts for the other staff.
Standard #: 8VAC20-780-80-A Description: Repeat Violation
Based on observation and record review, the center did not maintain a written record of daily attendance that documents the arrival of each child in care as it occurs.
Evidence: At the start of the inspection there were 10 children marked present on the daily attendance. There were 11 children present at the facility.Plan of Correction: Corrected during the inspection. I'm going to ask parents to start signing in and then the staff will have to do a second review of attendance.
Standard #: 8VAC20-780-320-B Description: Based on observation, childrens restrooms did not contain all required supplies.
Evidence: The childrens bathroom did not contain paper towels.Plan of Correction: Corrected during the inspection.
Standard #: 8VAC20-780-350-B-1 Description: Systemic Deficiency
Based on observation, the center did not follow a 1:4 ratio for children between birth months up to 16 months.
Evidence: At the start of the inspection, Child #1 (3 months) was observed in a classroom with 11 children supervised by 2 staff.Plan of Correction: Our infant teacher had an emergency and asked to come in late. I'm trying to get new staff.
Standard #: 8VAC20-780-500-A Description: Based on observation, hand washing requirements were not met.
Evidence: Child #1 was observed having their diaper changed. The child's hands were not washed after the diaper change.Plan of Correction: I'll remind staff of hand washing procedures.
