Technical Assistance:
Technical assistance was given licensing standards regarding linens and outdoor play were reviewed.

Comments:
An unannounced monitoring inspection was conducted on October 16, 2023. The inspector arrived at 11am and departed at approximately 1:40PM. There were 36 children present and 9 staff members. A sample size of ten records were reviewed which included five children?s records and five staff records. The Inspector observed staff and children?s interactions throughout the inspection. Children were observed during group activity, free play both indoor and outdoor, lunch, and naptime. First aid kits and emergency preparedness documents were observed. Medications were observed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented. The violations are listed on the violation notice issued to the center and were reviewed with the site owners/director at the exit interview.

Violations:
Standard #: 8VAC20-780-270-A
Description: Based on observation and interview, it was determined that the center did not ensure that areas and equipment of the center, inside and outside, be maintained in a safe and operable condition.
Evidence:
1. There are the following concerns on the outdoor playground of the center:
a. The pink Little Tykes car has two protruding screws on the front of the car that pose as a snagging and injury hazard to the children in care;
b. The large black tire that sits on the ground has accumulated water inside of the rim which poses concerns of stagnant water to attracts various bugs or animals to harm the children in care;
c. The blue truck that children climb on has a missing steering wheel and there is a 1 inch screw that is exposed and loose. This poses as a possible snagging, choking, or injury hazard to the children in care;
d. The plastic picnic table and bench set, which is the height and size of the children in care, is broken as it has loose parts to the table and very unstable. Should children attempt to sit on the bench, it will immediately fall and it poses as an injury hazard.
2. Staff #1, Staff #2, and Staff #3 confirmed that the playground concerns listed above are broken and are in need of repair or replacement.

Plan of Correction: The center responded with the following: The center will put in a work order to repair all the equipment of concern on the playground. The equipment that poses hazard or concern to the children will not be used until safe for children to use.

Standard #: 8VAC20-780-550-D
Description: Based on record review and interview, it was determined that the center did not ensure they implement a monthly practice evacuation drill.
Evidence:
1. The practice evacuation drill log was observed and there is no practice drill conducted the month of September 2023.
2. Staff #2 confirmed that there was no practice evacuation drill conducted for the month of September 2023.

Plan of Correction: The center responded with the following: The center will ensure that all practice evacuation drills will be implemented monthly and documented.
