As discussed, children under the age of 3 years must be served food that is cut in size to prevent choking.

Discussed equipment in the Red Room and the wear and tear on furnishings. Please be mindful of wooden equipment and the risk of splintering.

Per our conversation, the fall zones on the playground must be free from obstacles. Please be mindful of the portable play houses and the location in which they are placed.

Comments:
An unannounced Monitoring Inspection was conducted today. A total of 26 children with 4 staff and 4 co-op parents were present. Additional management staff and float staff were also present. The center was celebrating their fall festival today. The children were observed decorating pumpkins by gluing eyes, yarn and pom poms on them. A second activity had the children decorate sugar cookies with sprinkles and icing and then eating them. Some children were observed in the hay pile where they were able to search for a prize. All four classrooms rotated outside to the fall activities. The children were observed transitioning inside where they lined up for the restroom and handwashing. A group of children were upstairs having morning snack under the supervision of staff and volunteers. One medication onsite and was reviewed. A total of 5 children's records, 2 staff and 3 volunteer records were reviewed. This inspection occurred between 10:15am and 12:10pm. Thank you for your cooperation today. Standard #: 22.1-289.035-B-4
Description: Based on staff record review, background checks for one staff were not obtained prior to volunteering.
Evidence:
Volunteer #1 has lived outside of the state of Virginia in the past 5 years. An out of state Central Registry search and an out of state criminal background search has not been obtained.

Plan of Correction: The background checks will be requested immediately and the results will be forwarded to the Licensing Inspector for review. This volunteer will not be onsite until all background checks are received.

Description: Based on staff record review, one staff record was incomplete.
Evidence:
Staff #2 is in a lead teacher capacity. Documentation to demonstrate that the individual possesses the education, certification for the position was not maintained.

Plan of Correction: Documentation of staff qualifications will be obtained and maintained in their file.

Description: Based on review of medications, the center failed to follow their own medication policy.
Evidence:
The medication authorization for a long term prescribed medication for Child #1 was not signed by a physician.

Plan of Correction: The form will be given to the parent to have the physician sign the authorization.
