A self-reported other monitoring inspection was initiated on 10/19/23 and concluded on 10/19/23. The inspector reviewed compliance in the area of programs. The inspection started at 9:00am and concluded at 9:30am.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-780-410-5 Description: Based on review of video and discussions with staff the center failed to ensure that children were not force fed.
Evidence:
On 10/05/2023 a 19 month old child was at the lunch table with staff #1 sitting beside him. Video showed staff #1 attempt to force feed the child approximately seven (7) times with each attempt being more physical. The child's head could be seen moving backwards with each attempt. The child was also captured on video pushing his hands against the table each time, causing the table to slide twice.Plan of Correction: Witnessing staff reported this to the assistant director, who reported it to the director. The child's mother was immediately notified as well. The director watched the video and then fired staff #1. Also, all staff have been required to retake the Child Abuse training.
