Technical Assistance:
Consultation provided on the additional components of orientation training as of 10/21. Please review standard 240. C ( 1-11).

Comments:
An unannounced renewal inspection was conducted with the morning director, who was present with two additional staff and four children. The inspection took place from 7:10 am to 8:05 am and the ages of the children ranged from 5 to 10 years. The children were doing a "art around the world" project which consisted of viewing a video of ivory carvings from Nigeria. Two children had medications on site and one child had a diagnosed food allergy . Four staff and six children's files were reviewed during this inspection. The areas of non compliance are outlined on the violation notice.

Violations:
Standard #: 22.1-289.036-A
Description: Based on interviews, it was determined that 3 agents did not have repeat fingerprint background checks every five years.
Evidence:
Person A, B and C's fingerprint background checks were completed on 9/20/18 and had not been renewed within 5 years of that date.

Plan of Correction: We obtained the repeat background checks on 10/19/23.

Standard #: 8VAC20-780-510-E
Description: Based on a review of medication compliance for two children and interview with Administration, it was determined that there was no parental authorization for two medications for one child.
Evidence:
There was an antihistamine and a medication to treat a respiratory condition on site for child A, although both medications had a physician authorization, neither had a parental authorization.

Plan of Correction: We will obtain the parental authorizations.
