Comments:
An unannounced monitoring inspection was conducted on October 12, 2023, from 11:20 a.m. through 1:10p.m. Upon arrival, there were 36 children, ages 4 months through 4, in the care of 12 staff.

The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. Five children and twelve staff records were reviewed.

Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Standard #: 22.1-289.058
Description: Based on observation the facility failed to have a carbon monoxide detector.

Evidence:

1. Observation of the classrooms revealed a carbon monoxide detector was not installed.
2. The director confirmed the center did not have a carbon monoxide detector.

Plan of Correction: Carbon monoxide detectors were immediately ordered.

Standard #: 8VAC20-780-60-A
Description: Based on record review the facility failed to have documentation of viewing proof of identity for one child.

1. Five children records were reviewed.
2. Child #2 did not have documentation of viewing proof identity.
3. The director confirmed proof of identity had not been documented for Child #2.

Plan of Correction: Proof of identity will immediately be requested from the parent and documented.
