A monitoring inspection was initiated on 10/24/2023 and concluded on 10/24/2023 from 11:00 AM to 1:20 PM. There were 13 children present, ranging in ages from three to five, with 4. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies. A total of five children?s records and the three staff?s records were reviewed.
Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Standard #: 8VAC20-780-130-A Description: Based on record review and interview, the center failed to obtain immunizations for a child prior to attending the center.
Evidence:
1. The record for child 2 was reviewed. The first date of attendance was 8/9/23. There was no documentation of immunizations in child 2's record.
2. Staff 2 checked with the main office to ensure the documentation had not been received. The document was provided dated as received 10/24/23.Plan of Correction: The immunizations were obtained for child 2 during the inspection. In the future the center will ensure immunizations are obtained prior to attendance at the center.
