Comments:

AN unannounced monitoring inspection was conducted on 10/23/23, from 2:40 to 3:25. The center had 10 children in care with one staff at first, a second staff arrived 25 minutes after the children arrived. The children were observed playing lots of games in the gym, later they were observed working on chrome books and eating snack. Today's snack was cinnamon toast crunch and milk. Lots of interaction between staff and children. A violation notice was given to the staff along with this inspection summary. Reviewed 3 children's records during the inspection. Description:	Based on the review of children's records, the center failed to get some information required to make the child's file complete. Evidence: Child 1, is attending form another school and does not have immunizations on file.
Plan of Correction:	I believe we have this information at the office , I will ask them to send it to us.
Standard #:	8VAC20-780-140-A
Description:	Based on the review of 3 children's records, the center failed to get some of the required information to make the child's record complete. Evidence Child 1 is missing the physical, the child attends a different school and the physical is not available at the location.
Plan of Correction:	I will get this information from the YMCA office
Standard #:	8VAC20-780-70
Description:	Based on the review of staff records available on site, the center failed to have some of the required paperwork available. Evidence: The center does not have the emergency contact information for staff on site.
Plan of Correction:	We filled this out at the beginning of the year but turned it in to the office I will get a copy from them.
Standard #:	8VAC20-780-340-C
Description:	Based on the observation and conversation with staff, the center failed to have two staff present during the inspection. Evidence: When the inspector arrived at school dismissal, 14:40, the center had one staff present. 10 children arrived for care. The second staff arrive 25 minutes later at 15:05. For the 25 minutes only one staff was present with the children.
Plan of Correction:	I will talk to the YMCA office and let them know we need to people here every day as soon as we have children in care.
